MDS Coordinator/Nurse Educator

Jefferson City Manor is seeking a dynamic RN to join our leadership team as an MDS Coordinator/ Nurse Educator! Jefferson City Manor Care Center is a newly remodeled nursing facility that provides excellent skilled nursing care, rehab services, respite, and hospice care. The MDS Coordinator works alongside the MDS Director who manages the PPS MDS, while the MDS Coordinator manages the long-term care residents. The MDS Coordinator/ Nurse Educator will split their time between the MDS process for long-term care residents and staff education and training; implementing educational and professional development activities and programs that optimize resident outcomes.
The MDS Coordinator/Nurse Educator responsibilities include preparing, completing, and managing the MDS process for the long-term care population including individualized plans of care from admission to discharge. Accurate and timely completion and submission of MDS assessments to meet state and federal regulations. The MDS Coordinator will also function as a nurse educator providing staff education, training, and orientation, CNA instruction, and clinical in-servicing.
Qualifications include:
• Undisciplined RN license
• CNA instructor certification (or willingness to become certified)
• MDS 3.0 experience
• Familiar with CMS payment and reimbursement practices
• Familiar with the use of the Residential Assistance Instrument (RAI) per OBRA.
• Excellent oral and written communication skills and the ability to motivate.
