MDS Coordinator

Direct, coordinate and facilitate the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care for residents.

Responsibilities
1. MDS/PLAN OF CARE ASSESSMENT FUNCTIONS
Perform resident clinical and mental assessment. Collect data regarding problems and strengths, goals and approaches for nursing care of each resident that is consistent with Centers for Medicare and Medicaid (CMS) guidelines and company standards utilizing the RAI process to formulate a pertinent Plan of Care.
Maintain current MDS status of each resident, according to state and federal guidelines as required under the Omnibus Budget Reconciliation Act (OBRA), Medicare PPS and or Medicaid required assessment and implements a system to monitor for changes in resident condition.
Maintain concurrent interface with Managed Care Companies to facilitate reimbursement from third party payor sources as required.
Identify and address needs required for assistance with activities of daily living and nursing diagnoses with goals and objectives.
Attend morning meeting Monday – Friday to be aware of any changes in condition of residents and collaborate concurrently with designated charge nurse on each neighborhood to facilitate alteration in approaches to assure favorable outcomes.
Be aware of condition changes of residents and perform required MDS assessments within required time frames as defined by regulatory requirements and associated MDS Assessment Reference Dates (ARD)
Submit MDS within designated time frames established by regulatory bodies.
Identify possible goals of staff, resident and family.
Using nursing interventions correlate all data regarding each resident individually.
Coordinate and lead Plan of Care meetings with the multidiscipline team for a comprehensive plan of care. Team members may include the following disciplines: Charge nurse, Certified Nurse Assistant, PT, OT, ST, Social Services, Chaplaincy, Dietary, Activities, resident and resident family.
Assist Charge Nurse on following through as necessary.
Keep all care plans up to date so they can be used as guidelines for staff on the neighborhoods.
Review care plans quarterly and facilitate the quarterly meetings with interdisciplinary team, families and residents to ensure that appropriate care is coordinated, delivered and documented accordingly.
Keep track and ensure accurate dates for annual and quarterly MDS and care plan updates; notify care plan team of the same.
Ensure that nurses’ notes reflect that the care plan is being followed.
Ensure that assigned nursing assistants are aware of the resident care plans and that they are used in administering daily care to the resident.
Maintain confidentiality of all resident care information.

2. ADMINISTRATIVE FUNCTIONS
Participate in developing, maintaining and updating of our written policies and procedures that govern the day-to-day functions of the Nursing Services Department.
Ensure that the Nursing Service Procedures Manual is followed by nursing service personnel, and that it reflects the day-to-day nursing care procedures used by the Community.
Assist in developing methods for coordinating nursing services with other resident services to ensure the continuity of the residents’ total plan of care.
Participate in the development, maintenance and implementation of the community’s quality assurance program for the Nursing Services Department.
Periodically review the resident’s written move out plan. Participate in the updating of the resident’s discharge plan as required.
Participate in survey (inspection) made by authorized government agencies.
Make written and oral reports/recommendations to the Associate Director of Nursing, Director of Nursing and Administrator as necessary/ required, concerning the operation of the MDS/Care Plan process.. .

3. REQUIREMENTS/CHARTING/ DOCUMENTATION
Maintain current thorough knowledge of MDS 3.0 and subsequent regulatory updates, RAI and care assessment processes.
Fill out and complete accident/incident reports, if involved. Submit to Director of Nursing as required.
Chart all accident/ incidents involving the resident, if involved. Follow all established procedures.

4. PERSONNEL FUNCTIONS
Develop and maintain a good working rapport with inter department personnel, as well as other departments within the facility to assure that nursing services and activities can be adequately maintained to meet the needs of the residents.

5. STAFF DEVELOPMENT
Participate in an effective orientation program that orients the new employee to your shift, its policies and procedures, and to his/her job duties.
Attend and participate in continuing educational programs designed to keep you abreast of changes in your profession, as well as to maintain your license.

6. SAFETY AND SANITATION
Ensure that your assigned work areas are maintained in a clean and sanitary manner.

7. RESIDENT RIGHTS
Follow all Resident Rights policies and procedures in the employee handbook.
Maintain the confidentiality of all resident care information.
Ensure that all nursing service personnel are knowledgeable of the resident rights and responsibilities.
Maintain CONFIDENTIALITY of all pertinent resident and employee information to assure their privacy is protected.
Support and uphold all federal, state and local regulatory laws and standards.
Support and uphold all Lutheran Senior Services Policies and Procedures.
