The Clinical Nurse Leader is responsible for the planning, implementation, and evaluation of resident centered care planning, compliance, development of all nursing service personnel, performance management, evaluation, monitoring staffing needs, policy and procedures, and communication with resident and family members. Plan, develop, organize, implement and evaluate Nursing Services program in the Rehabilitation Unit. Participate in pre-admission assessment data collection. Manage admissions, transfers, and discharge residents as required. Monitor and evaluate staff development needs including orientation, performance management, and leading of self-directed work teams. Empower staff members to participate and promote Culture Change.
