Purpose:  Effectively involving residents and families in shared decision making is essential for improving outcomes and achieving goals of care. Clinical staff must rely on advance care planning processes and skills to complement legal forms and papers to ensure that patient preferences are honored.  End of life decision making is a shared duty between patient, provider and proxy.
